Where flights come from, before they are trips. Most arrive from partners who see the patient first: hospital social work, nurse navigators, patient-support organizations. The rest come through the ACA website, our own site, or a phone call. Pick a row to review it.

Custody: ACA holds website requests until a member accepts. Partner links land in our custody directly.Missing counts what we need before accepting is sensible; accepting with open items is allowed and they carry onto the trip.

Decision
Accept now so the relay can be arranged; the release carries over as an open item. How requests reach us
Partner link
One link per partner organization. Pre-fills the organization, verifies the submitter against their staff list, carries their attestation and release. Lands in our custody. Most of our volume.
ACA website
The public form on aircharitynetwork.org. ACA holds the request and proposes a member by region and mission type. We accept, decline or reroute. Partners sometimes use it instead of our link; it still works, with one extra step.
Our website and phone
Self-referred. Ours from the start, verified by nobody. The first move is usually to find the partner behind it: the clinic that can attest and send the release.
Other members
Referral, relay or transfer from another ACA member under a sharing agreement. Scope is set by the agreement, not by us.

Self-referred requests take four times longer to decide and are declined twice as often. Most of that gap is finding the clinic.
Contacts, by rule
Every request names a primary passenger.
The person travelling for care. Weight, needs and documents hang off them.
Every request names a primary contact.
The passenger, or a caregiver. A minor is never the contact.
A caregiver is travelling or not.
Travelling: counted in seats and weight, signs their own waiver. Not travelling: a phone number and a role, no seat, no documents, labelled as such everywhere.
Referrers stay scoped.
They see milestones for their own request and get document asks by link. Never the itinerary or the pilot.
